These care or nursing homes have been told that they must make drastic improvements by the health watchdog, the Care Quality Commission (CQC). The CQC rates care homes across five sub categories - Safe, Effective, Caring, Responsive and Well-led. The 'Inadequate' ranking is the lowest, usually sparking enforcement action by health officials. All of these have been deemed inadequate by the CQC in the 'Safe' and 'Well-led' categories with some getting the lowest rating in a third category. Kingston Nursing Home (7 Park Crescent, Leeds, LS8 1DH) Kingston Nursing Home Safe: Inadequate Effective: Good Caring: Good Responsive: Good Well-led: Inadequate The report reads: "People did not always receive safe care. "Before and during this inspection we identified and reported safeguarding concerns. People's medication was not always administered safely and we could not be sure all staff administering prescribed medication were competent to do so. "Risks to people's care were not always managed safely and staff's knowledge was not robust. "During this inspection, we could not be sure people would always be appropriately supported in the event of a fire. "The environment was not always clean and free of odours. "Staff were not recruited safely." The Gateway Care Home (1 Sticker Lane, Bradford, BD4 8RD) The Gateway Care Home Safe: Inadequate Effective: Good Caring: Good Responsive: Good Well-led: Inadequate The report reads: "People were not safe. Risks to individuals were not appropriately assessed and managed. Medicines were not managed safely. Lessons were not learned when things went wrong. "The provider's quality management systems were not effective and did not identify areas where the service needed to improve. The provider and registered manager did not demonstrate they understood their legal responsibilities. “People who used the service, relatives and staff provided consistent positive feedback about their experience. The management team were responsive to the inspection findings and shared plans to improve their systems and processes. "Safeguarding procedures were not consistently followed. There were sufficient staff to keep people safe and they usually worked in the same unit, so people received continuity of care. People lived in a safe, clean and pleasant environment." The Gateway Respite (580 Sticker Lane, Bradford, BD4 8RD) Safe: Inadequate Effective: Good Caring: Good Responsive: Inadequate Well-led: Inadequate The report has not yet been posted. The Glynn Residential Home (167 Bradford Road, Wakefield, WF1 2AS) This Wakefield came is one of eight in West Yorkshire ranked as inadequate Safe: Inadequate Effective: Good Caring: Good Responsive: Requires Improvement Well-led: Inadequate The report reads: "People were at risk of avoidable harm because risks had not been adequately assessed, monitored or mitigated. "Care records did not contain sufficient information for staff to know how to care for people safely or understand their individual risks. "Staff had not received adequate safety related training. "Equipment was not assessed for individual use, or robustly checked to ensure people could use this safely. Systems and processes did not ensure the safe management of medicines or suitably skilled and trained staff. "Quality assurance processes were weak and no improvement had been made to address the breach at the last inspection or recognise further breaches in regulations we identified at this inspection.

Recruitment checks were not thorough and did not ensure staff were safe and suitable to work in the service. "There were not always enough staff to meet people’s needs and keep them safe. "There was a lack of effective and consistent management of the service. "The registered manager left in December 2018 and since then there have been three managers, none of whom have registered with CQC. "The provider recently introduced new quality assurance systems however these were not always effective in identifying and addressing areas where the service needed to improve. "People who used the service, relatives and staff provided consistent positive feedback about their experience. The provider and manager responded to the inspection findings and told us of some actions they had taken to improve the service." Southfield Care Home (Belton Close, Great Horton, Bradford, BD7 3LF) Four of the eight worst ranked care homes were in Bradford Safe: Inadequate Effective: Good Caring: Good Responsive: Good Well-led: Inadequate The report reads: "People were not always safe. Government guidance on the prevention and control of infection was not always followed which meant people were at risk of catching an infection. "None of the people using the service had Covid-19 when we visited. "The provider was supporting people and staff to have regular Covid-19 tests. "Relatives of people using the service told us they had been supported to keep in touch during the pandemic." Whitwood Grange (Smawthorne Lane, Castleford, WF10 4ES) Safe: Inadequate Effective: Good Caring: Good Responsive: Good Well-led: Inadequate The report reads: "We found a lack of oversight over key aspects of the service such as the premises and infection control, assessment of people’s capacity, formal staff support and outcomes for people living in the home. Some records we requested from the provider to demonstrate their quality oversight were not available. "Some areas of the premises were in need of repair. Infection control standards required improvement as staff were not consistently using PPE correctly and supplies of toiletries were not available. "People received their medication as prescribed, although storage arrangements and some recording was not adequate. "People were not supported to have maximum choice and control of their lives and staff did not support them in the least restrictive way possible and in their best interests; the policies and systems in the service did not support this practice. "Assessments of people’s capacity had not been appropriately completed. There was no evidence to show these decisions had involved people’s representatives and they had not been reviewed." Moor View (20 Georges Square, Nursery Lane, Halifax, HX3 5TA) Safe: Inadequate Effective: Requires Improvement Caring: Requires Improvement Responsive: Requires Improvement Well-led: Inadequate The report reads: "People were not always safe. Medicines were not always managed safely and staff did not feel adequately trained to manage medicines. There was evidence of staff working outside the registration of the service in relation to administration of medicines. "Some staff lacked confidence in developing and understanding risk assessments. "Not all staff understood how the emergency call system worked and staff had failed to report issues with the system. "Staff had not been provided with all the training they needed to carry out their duties and were not always supported in their roles."
